How to Become a Patient
Becoming a Patient
When you schedule your initial office visit, you will be asked to create an account on the Patient Portal or to download and fill out our new patient forms below, and bring them in with you.
We also prefer that you bring any medication and supplements that you are currently taking with you to this initial visit so we know exactly what you are taking.
We also ask that you bring current lab work with you (within the last 6 months). If you do not have any copies, we are happy to have you fill out a records release and get those at a later date.
Please arrive 10 minutes before your visit so we can get your paperwork ready.
Allow up to 90 minutes for your first appointment.
During your Initial Appointment
At this time the doctors will do the following:
Get the big picture and gather all of your information from many different aspects.
Discuss your goals and expectations.
Take an initial health history with your current symptoms.
We will do a review of organ systems to inventory all imbalances from each organ system.
Review any labs that you are able to bring in with you from a Naturopathic view point.
Review all of your medications and supplements.
Review your diet, lifestyle choices and stressors.
Perform any physical exams necessary.
Discuss together what future lab work is needed and why.
Discuss together what next initial lifestyle changes you can take until results of labs are in.
We Work With you!
Our naturopathic doctors, holistic nurse coach and our nutritionist, work for and with you to find the treatments that are right for you. There can be many ways to address the imbalances that you are experiencing and our goal is to try to find the treatment that is right for you and the treatment that is do-able for you.
We are THRILLED to announce that we are now using an EMR system (electronic medical records) called Power 2 Patient for your medical profile and e-sign documents. This portal allows our patients to take control of their health by being able to contact your doctor, access lab results, get prescription refills along with many other features accessible on YOUR time. Call our office to schedule an appointment and we will send you an email on how to register.
If you do not have an email you are welcome to click on the links below to print your new patent documents and bring them in with you.
